Accepted referrals
Children and adolescents aged 0-18 years may be referred for specialist assessment of seizures and epilepsy, developmental delay or regression, cerebral palsy and spasticity, hypotonia or weakness, neuromuscular disorders, movement disorders, headache and migraine, sleep-related neurological concerns, neurogenetic conditions, neuroinflammatory disease and other complex paediatric neurological presentations.
Neurophysiology requests
- Routine, sleep-deprived, prolonged, video and ambulatory EEG
- Nerve conduction studies and EMG
- Visual and auditory evoked potentials
- Other clinically appropriate paediatric neurophysiology
- Paediatric polysomnography (sleep study)
Information to include
- Patient identifiers, caregiver contact details and medical-aid information where relevant
- Clear clinical question, onset, course and important examination findings
- Current medicines, allergies and relevant comorbidities
- Previous EEG, imaging, laboratory, genetic or sleep-study results
- Urgency and the reason it cannot follow the routine pathway
- Referrer name, practice number, contact details and preferred report route
How to refer
Email the completed form to receptionist@draajalloh.com. Please send only information needed for the referral and use your organisation’s approved secure method for confidential records. Reception can confirm receipt and the administrative pathway.

Reports
Reports are communicated securely to the referring clinician using the contact information supplied. Please ensure the requested report route is current and belongs to the treating professional or authorised organisation.
